She went to medical school at AUC and completed her undergraduate studies at UT Austin where she obtained her Bachelor's degree in Biology.

Dr. Azeem specializes in the evaluation and management of neck and back pain, non-operative spine care and musculoskeletal injuries. She performs fluoroscopically-guided interventional spine procedures and joint/soft tissue injections along with medical management to treat pain.

She is board-certified in Physical Medicine & Rehabilitation and is fellowship trained in Interventional Pain and Spine.

Louis Cornacchia, MD

Dr. Louis G. Cornacchia is a board-certified neurosurgeon who was born in the Bronx, NY. He completed his BA at Columbia College, his MD at NYU School of Medicine, and his neurosurgical residency at the University of California, San Diego, with an in-residence in joint orthopedic and neurosurgical spine. Since completing his residency, Dr. Cornacchia has practiced neurosurgery in Long Island, Queens, and Brooklyn for over twenty years in a variety of different positions.

Dr. Cornacchia was Chief of Neurosurgery at Mercy Medical Center from 2001 - 2006, Hempstead General Medical Center and Brookdale University Medical Center from 2001-2016. He was a Clinical Assistant Professor of Neurosurgery at New York Presbyterian Cornell from 2009. Later, Dr. Cornacchia served as staff surgeon at NY Presbyterian Queens and Assistant Professor of Neurosurgery at Weill-Cornell Medical center.

Dr. Cornacchia is the author of numerous neurosurgery and neuroscience research articles and the author of almost two dozen patents. His clinical focus at this time is improving outcomes in complex spine neurosurgery and his research focus is on improving the safety and outcomes of surgery by improving surgical ergonomics.

Ronald Daly, MD

Mark Goodstein, MD
Pain Management

Mark Goodstein, M.D. is a double board-certified Anesthesiologist and Pain Management Physician. He specializes in interventional management of spine pain with a focus on reducing pain and restoring the function of physical capacities. He has been practicing for over 20 years. He received his undergraduate degree from Washington University in St. Louis and his medical degree from Boston University School of Medicine. He completed his fellowship in Pain Management from Thomas Jefferson Medical Center in Philadelphia.

Alexandre Grigorian, DO

Dr. Alexandre Grigorian obtained his graduate medical degree from North-West State Medical University, St Petersburg, Russia in 1990. After relocating to the US, he obtained his second graduate medical degree from New York College of Osteopathic Medicine in 1998 and completed Family Practice residency program at Wyckoff Height Medical Center in 2001. Dr. Grigorian is a Diplomate of the American Board of Family Physicians and authorized and registered with NYS Workers' Compensation Board. Dr. Grigorian is fluent in English and Russian.

Michael Jurkowich, MD
Pain Management, Physical Medicine & Rehabilitation

Dr. Michael Jurkowich is one of our physicians and is a board-certified Physiatrist as well as fellowship-trained in Interventional Spine and Sports Medicine. He completed his fellowship training at Alabama Orthopedic Spine and Sports Medicine Associates.

Prior to this, he completed his residency training at the State University of New York, Downstate, serving as Chief Resident in his final year.

Dr. Jurkowich attended Ross University School of Medicine where he graduated with High Honors. He currently remains an active member in multiple medical organizations including the Spine Intervention Society, American Academy of Physical Medicine & Rehabilitation, American College of Sports Medicine, and the New York Society of Physical Medicine and Rehabilitation.

Dr. Jurkowich specializes in non-surgical treatment of headaches, neck and back pain through the use of epidurals, facet joint injections, sacroiliac joint injections, radiofrequency ablation, vertebral augmentation, spinal cord stimulator trials, discography, and intra-discal steroid injections. Through his extensive training, Dr. Jurkowich is also certified to perform electrodiagnostic testing such as EMG/NCV which can prove vital in the diagnosis and targeted treatment of such conditions.

Additionally, Dr. Jurkowich employs the technology of ultrasound to perform MSK examinations as well as peripheral joint and tendon injections for the treatment of a wide range of musculoskeletal injuries.

Oleg Olshanetskiy, DO

Dr. Olshanetskiy, received his first graduate medical degree from Odessa State School of Medicine, Ukraine in 1991. He received his second graduate medical degree at New York College of Osteopathic Medicine, Old Westbury, N.Y. in 1998. He completed a 3 year Family Medicine residency at St. John’s Episcopal Hospital of the Mount Sinai Healthcare System, New York and Peninsula Hospital Medical Center of the Northwell LIJ Healthcare System, New York.

Dr. Olshanetskiy serves as a Senior Aviation Medical Examiner for the F.A.A. and a Certified Medical Examiner for the D.O.T. He is honorably recognized by the American College of Family Physicians and Diplomate of American Board of Family Physicians. Dr. Olshanetskiy is fluent in Russian, Ukranian, and English.

Nazia Shah, MD

Nazia Shah, DPM, is an associate of the American College of Foot and Ankle Surgeons, (ACFAS). Dr. Shah graduated from Temple University and finished her surgical training at Hoboken University, New Jersey.

Dr. Shah has gained training in all aspects of foot and ankle surgery with a primary focus on reconstruction, acute and chronic trauma, and sports injuries.

She is an enthusiast. Her passion lies in tailoring treatment based on each individual. She excels in being intuitive to all patient concerns. Her primary focus is patients’ comfort and to provide education to make it easy to appreciate the root of his or her pain. Each day when a patient walks in relaying symptom-free ambulation is a good day.
